Insurance Premiums

If for any reason you have a hardship paying your Club Life Insurance, Long Term Care, Long Term Disability, or any other Club-related premiums, please contact us immediately at (800) 464-0452 x 121 or at

Insurance Claims

The Club’s claims department is available by phone and through email to help you complete your claim. If you have the Club’s Group Rated Life Insurance, Long Term Disability, Long Term Care, Accident, or any other Club insurance product, please contact Monica Zamudio at (800) 464-0452 x 122 or at


Club Store Online

While the Club’s physical store remains closed, the online Club Store is open, but because of the City’s Safer at Home directive, we are unable to ship online merchandise at this time.
